BTE Consent & Acknowledgements Uploads

  • Welcome BTE Family!

    To ensure compliance with global BTE consent and acknowledgement policies, all BTE site coordinators are asked to upload signed copies of the BTE photo and evaluation consent forms for each BTE participant, as well as the signed Data Sharing Agreement, signed by the school or community based organization partner that allows for key data to be shared to allow BTE to conduct evaluation activities in the program's pursuit of greater impact for all BTE beneficiaries. These forms were included in the BTE Acceptance Packet all sites should issue when accepting youth into the BTE program. If utilizing the BTE mobile app, signed consent forms should also be uploaded (Please note, use of the BTE mobile app is optional and not required). Please use the following form to submit your file uploads.
  • Please upload all signed photo and evaluation consent for each BTE participant. These forms can be found in the BTE Acceptance Packet.
