    We are asking you to take part in an evaluation of the Pathway to Success program. We are interested in learning about your involvement and role in the program, your perspectives about how the program is working, strengths and challenges with the program, and any suggestions you might have about possible improvements or enhancements.

    You were identified as a volunteer participating in the Pathway to Success program who might provide useful comments and feedback. There may be no direct benefit to you in participating, however, your comments will help us, the evaluators, and program staff, better understand how well the program is working, and possible ways that the program might be improved.

    Your individual information will be treated confidentially.
